You should first download this form to your computer so that you always have an electronic copy.
To do so click on the menu FILE --SAVE AS and then choose a folder on your hard disk from the
dialog box which appears. Call this file Newform and in FILE TYPE select html.

Once the form is downloaded you can work offline.
If your browser is Netscape than you simply call up the file and choose FILE EDIT
then proceed as detailed below to enter new members. Save this amended form and
then ATTACH IT TO AN EMAIL to mail at croquet-nsw.com where at is @
If you are using Internet Explorer then you can proceed in the same way or you can edit the form
in Microsoft Word and attach the saved copy of Microsoft Word to an email

YOU WILL ALSO NEED TO SEND A HARD COPY OF THIS FORM BY SNAIL MAIL WITH YOUR CHEQUE
BUT PLEASE ASSIST US BY COMPLETING THE FORM BELOW AND ATTACHING TO AN EMAIL AS
WE CAN THEN SCAN YOUR ENTRY DIRECT INTO OUR DATA BASE OF MEMBERS

Notes:   You can add more rows once you have downloaded should you need to do so
    column 1 – enter here the suggested handicap of the new member
    column 2 - enter the surname of the member in CAPITAL LETTERS
    column 3 - enter the Title of the member Mr, Mrs, Ms , Dr etc
    column 4 - enter the first name ONLY in small case
    column 5 – optional – enter the year of birth e.g 34 if born in 1934, 29 if born in 1929 etc
    column 6 – enter the telephone number WITHOUT THE (02code), AND BE CAREFUL TO PUT
    THE SPACE AFTER THE FIRST 4 NUMBERS
    column 7 – enter the Address – try to be economical in this to keep it short
    column 8 - enter the town with postcode in CAPITALS – NOTE THE SPACE before the postcode
    column 9 – enter Y if you are affiliating thorough your club and paying the fee. Enter N followed
    by the abbreviation for the club where the member is already affiliated e.g N BOO if the member
    is NOT affiliating through your club but you know he/she
    is affiliated through Boolaroo – note the space between N and BOO
    ( consult the Yearbook for other examples).
      FINALLY ---IF YOU FIND THAT SOMETHING DOES NOT FIT JUST TYPE AND THE ROW
        WILL EXPAND TO ANOTHER LINE
